Don't forget about Adobe Flash and After Effects!
Two GREAT programs.

I use on windows: -NotePad++, this for quick edits, or siuations where I dont want to load up dreamweaver or some other large suite. It is quick, light weight loads fast. Supports tabs, and syntax highlighting.
-Dreamweaver.. This for pretty much every thing creating the website.. except the graphics.
-Photoshop, For the graphics, a general UI idea etc.
-Fireworks, when I am lazy... Ties in with dream weaver much easier.. But sometimes produces some messy code.
-FlashFXP for FTP, uploading etc.
For flash graphics, I cant stand Adobe Flash(or what ever they call it now adays). It offers to much for what I need, and over complicates things. So I just use Swish. Great for simple flash animations things.
On my mac I pretty much use the same things, except Smultron instead of notpad++, and Transmission instead of flashFXP.
Also, depending on the site I use CuteNews for a content mangement system. It is not as advanced and feature rich as other things like Joomla.. But I like it because of it's simplicity.
